The player can use a knife and/or kick to make physical attacks in addition to shooting.

The majority of the enemy units have melee attacks because the player cannot be killed by enemy contact. The game's landscape can be destroyed in large portions, and sometimes doing so uncovers more goodies or power-ups.

The player comes across prisoners of war (POWs) throughout a level, and if they are freed, they grant the player benefits in the form of random objects or weaponry. The player obtains a scoring bonus dependent on the number of POWs rescued at the conclusion of each level. The number of POWs released goes back to zero if the player passes away before the level is through.

The branching path system is a brand-new feature in Metal Slug 3. In most missions, the player must select one of several forking paths, each of which has its own obstacles and varies in length and severity. The gameplay mechanics are the same as in previous Metal Slug games; in order to complete each level, the player must continuously shoot at a stream of adversaries. The player now faces a boss, who is typically much bigger and tougher than typical enemies. The player can find several weapon upgrades and "Metal Slug" tanks as they progress through each level. The player's offensive and defence are both significantly improved by the tank, known as the SV-001.
