The Game of Knights of Honor is played on a sizable map of Europe, extending to Africa's northern coast and Ireland to the Caucasus. Provinces are sized chunks of land on the map. Each province has a city as its capital and several "Rural Areas," including towns, farmland, monasteries, or coastal communities. These rural areas are distributed randomly over the map before every Game and cannot be changed. The city, however, has space for several structures that can benefit the countryside or the city, such as increasing religiosity in monasteries whenever a church is developed or increasing town income when a market is there.

Other structures, such as walls and towers, are required to protect the city or to recruit specific soldiers (such as a swordsmith for units who use swords or a fletcher for teams that use bows). There isn't much room for buildings in a city. As a result, not all structures may be constructed in every town. Therefore, the player must choose what to make for each municipality. Building constructions in KOH takes a while in contrast to other strategy games. To rule over all of Europe is the Game's primary objective. The player, however, is free to act in any way they like. There isn't a defined amount of time.
